An AI medical scribe for dermatology

What a dermatology note needs
- Lesion description: morphology, colour, border, size in mm, distribution, and exact anatomical site with side.
- Duration, change over time, itch, pain, bleeding, and triggers such as sun, products or occupation.
- Severity scores where relevant: PASI, EASI, DLQI, SCORAD, or body surface area.
- Treatments tried: topical potency, systemic agents, phototherapy sessions, dates and response.
- Dermoscopy findings in words, if you use dermoscopy.
- Screening before systemic therapy: pregnancy plans, bloods, infection screening.
Example: psoriasis review before systemic therapy
A SOAP note, Detailed. Invented patient.
S: Plaque psoriasis for 12 years. Flaring over 6 months. Itch disturbs sleep. Joint pain in both hands in the mornings, 30 minutes of stiffness. Used potent topical steroid and vitamin D analogue combination for 4 months with partial response. Completed 24 sessions of narrowband UVB last year; cleared then relapsed within 3 months. O: Plaques on elbows, knees, scalp and lower back. BSA about 14%. PASI 13.2. Nails: pitting on 4 fingernails. DLQI 15. A: Moderate to severe plaque psoriasis with nail involvement and possible inflammatory arthritis. P: Start methotrexate after baseline bloods and chest X-ray. Rheumatology referral for joint symptoms. Review in 8 weeks with PASI and DLQI.
PASI, DLQI and the number of phototherapy sessions are the figures an approval reviewer will look for.
Dermatology templates
The Dermatology group includes templates for phototherapy, psoriasis with systemic or biologic therapy, atopic dermatitis, skin biopsy, suspected skin cancer, severe acne and isotretinoin, hidradenitis suppurativa, chronic urticaria, vitiligo, alopecia, lesion removal, skin infection, autoimmune and blistering disease, patch testing, rosacea, leg ulcers and wounds, and complex excision or Mohs.
Excisions and biopsies fit the Procedure structure: site and side, lesion size, margin, closure, specimen sent, and aftercare.
Letters and approvals
Biologics for psoriasis, eczema and hidradenitis almost always need approval. See biologics prior authorization for what to document, then use the prior authorization letter mode. For eczema, the atopic dermatitis SOAP note shows a clean example.
From the same visit you can draft patient instructions for a treatment regimen or wound care, and a referral letter to plastic surgery or rheumatology.
A quick dermatology clinic workflow
- Before the visit. Pick the note structure. Many dermatologists use SOAP for medical visits and Procedure for excision lists.
- While examining. Name each lesion aloud as you look: site, side, size, morphology. The patient's answers about duration and itch are captured separately.
- After Stop. Read the draft, correct any site or size, then ask for patient instructions or a letter if needed.
- Paste. Copy the note into your EMR. Your history of notes stays in this browser only.
Reviewing a dermatology draft
- Site and side of every lesion. 'Left upper back' is not 'right upper back'.
- Sizes in mm and margins for excisions.
- Topical potency and product names; similar-sounding creams are common.
- Scores, which the draft writes as you said them.
- Photos. If you uploaded a report photo, check that no identifiers are visible in it.
What ClearPass does not do
- It does not replace your judgement. Every note is an AI draft that you review and correct before it is used.
- It is not a medical device and does not make clinical decisions. It is a documentation aid.
- It does not connect to your EMR. The note is copy-ready text that pastes into any system.
Privacy
Clinical note content is not stored on ClearPass servers: it is processed to write the note, then discarded. Consultation audio is transcribed and discarded, never stored. Your note history stays in your browser, on your device, and you choose how long it is kept. Common identifiers are removed from typed and transcribed text before it reaches the AI; that is a best-effort safeguard, so leave names, ID numbers and file numbers out where you can. Processing goes through the AI providers named in our Privacy Policy, which sets out exactly what each one receives and how long it keeps it. ClearPass does not sign HIPAA business associate agreements today.
Questions
Can I upload clinical photographs of the skin?
Photo upload is designed for reports, such as histology or lab results. ClearPass reads the clinical details from them. It does not diagnose from skin images.
Will it calculate PASI or EASI?
It writes the score you state. Calculate it with your usual tool and say the result.
Can it describe lesions for me?
It writes your description. Say the morphology words you want in the note.
Does it help with isotretinoin paperwork?
It drafts the note, patient instructions and any approval letter from what you documented. Programme-specific forms stay in your own system.
