An AI medical scribe for gastroenterology
Gastroenterology history is detailed and a little awkward: stool frequency and form, bleeding, weight loss, what was eaten and when. What the GI note needs to show
A gastroenterology note is read by the endoscopist, the pathologist, the surgeon and whoever reviews a funding request. The things they look for are consistent.
- Bowel habit in concrete terms: frequency per day, stool form (Bristol type), nocturnal symptoms, urgency, blood or mucus.
- Alarm features stated as present or absent: weight loss with amount and period, dysphagia, persistent vomiting, iron deficiency, family history of GI cancer.
- Upper GI symptoms: heartburn frequency, regurgitation, response to acid suppression and for how long it was tried.
- Liver: alcohol intake in units per week, risk factors for viral hepatitis, LFT pattern, fibroscan score, platelet count.
- IBD: disease extent and behaviour, current therapy and drug levels, faecal calprotectin, steroid courses in the last year.
- Endoscopy and histology results with location, size and number of lesions.

A Problem list note, Detailed. Invented patient and results.
1. Ulcerative colitis, left-sided (E2), diagnosed 4 years ago. Current: 3 to 4 stools per day, loose, blood in most, urgency, 1 nocturnal stool. No fever. Weight stable at 68 kg. On mesalazine 4 g daily and a second course of prednisolone this year, now tapering at 15 mg. Faecal calprotectin 640 ug/g. Hb 118 g/L, CRP 14. Plan: flexible sigmoidoscopy to assess activity. Screening bloods and chest X-ray before advanced therapy. Discussed options; patient prefers subcutaneous therapy. 2. Iron deficiency. Ferritin 12. Oral iron not tolerated. Plan: intravenous iron.
Steroid courses, calprotectin values and failed therapies are exactly what a biologic justification relies on, so they should be explicit in the note.
Gastroenterology templates
Typed cases can use the Gastroenterology group, which covers upper GI endoscopy, colonoscopy, H. pylori testing, endoscopic ultrasound, ERCP, IBD and biologics, reflux, abnormal LFTs, viral hepatitis B and C, pancreatitis, coeliac workup, capsule endoscopy, fatty liver and fibroscan, IBS, GI bleeding, dysphagia and bariatric referral.
For endoscopy lists, the Procedure structure gives a clean procedure note: indication, sedation, extent reached, findings, samples taken and follow-up.
Paperwork after the visit
From the same consultation ClearPass drafts patient instructions such as bowel preparation reminders or a low-FODMAP starting point, a referral letter to surgery or hepatology, and a sick note during a flare.

Reviewing a GI draft
- Location words. Sigmoid versus caecum, antrum versus fundus, and the distance in cm for colonic lesions.
- Polyp counts and sizes, and which ones were removed.
- Drug names: mesalazine versus methotrexate, and brand names for biologics.
- Negatives for alarm features, which protect you and inform triage.
- Units for calprotectin and alcohol, and whether a value is from today or the last visit.
The numbers check marks any figure the draft contains that was not said in the transcript.
What ClearPass does not do
- It does not replace your judgement. Every note is an AI draft that you review and correct before it is used.

- It does not connect to your EMR. The note is copy-ready text that pastes into any system.
Privacy
Clinical note content is not stored on ClearPass servers: it is processed to write the note, then discarded. Consultation audio is transcribed and discarded, never stored. Your note history stays in your browser, on your device, and you choose how long it is kept. Common identifiers are removed from typed and transcribed text before it reaches the AI; that is a best-effort safeguard, so leave names, ID numbers and file numbers out where you can. Processing goes through the AI providers named in our Privacy Policy, which sets out exactly what each one receives and how long it keeps it. ClearPass does not sign HIPAA business associate agreements today.
Questions
Can I use it for endoscopy reports?
Yes, as a documentation aid. Dictate or type the findings and choose the Procedure structure. It does not connect to endoscopy software; copy the text into your system.
Will patients be comfortable discussing bowel symptoms with it running?
You confirm the patient has agreed every time before transcription starts. Audio is transcribed and discarded, not stored. Explaining that usually helps.
Can it read a histology or fibroscan report?
Photograph the report and upload it; the clinical details are read into the note. Remove names and identifiers from the photo first.
Does it suggest ICD-10 codes for IBD and GERD?
It suggests ICD-10 and procedure codes where your system uses them. You confirm each code before using it.
