An AI medical scribe for pulmonology

Respiratory clinics depend on counts and measurements: exacerbations in the last year, courses of oral steroids, FEV1 and its percentage predicted, oxygen saturation at rest and on walking. ClearPass transcribes the consultation, writes the English note, and marks any figure that nobody said, so you can confirm the numbers that drive the plan.

What a respiratory note must carry

Whether the visit is for asthma, COPD, a nodule or interstitial disease, the reader looks for the same backbone.

Example: severe asthma review

A Follow-up note, Detailed. Invented patient and results.

Follow-up note (excerpt)
Severe asthma. On high-dose ICS/LABA, LAMA added 6 months ago, montelukast. Inhaler technique checked today: good. Pharmacy refill data show regular collection.
Exacerbations in the last 12 months: 4, each needing prednisolone. One admission, no ICU.
ACT 13. Night waking 3 nights a week.
Non-smoker. No occupational exposure. Nasal polyps, previous sinus surgery.
Spirometry: FEV1 1.9 L (62% predicted), FEV1/FVC 0.64, 14% reversibility.
Blood eosinophils 0.52 x10^9/L. FeNO 58 ppb.
Impression: severe eosinophilic asthma, uncontrolled despite optimised inhaled therapy and good adherence.
Plan: apply for anti-IL-5 biologic. Written asthma action plan updated. Review in 3 months.
Invented example for illustration. Not a real patient.

Every item a biologic request asks for is in that note: exacerbation count, steroid courses, adherence, eosinophils, and optimised background therapy.

Pulmonology templates

The Pulmonology group has guided templates for spirometry and PFTs, CT chest, severe asthma and biologics, COPD management, sleep study for OSA, bronchoscopy, pulmonary nodule, ILD and pulmonary fibrosis, pleural effusion, haemoptysis, chronic cough, pulmonary embolism workup, bronchiectasis, TB workup, lung cancer staging, pulmonary hypertension, home oxygen and NIV assessment, and occupational lung disease.

Bronchoscopy and pleural procedures fit the Procedure structure: indication, sedation, airway findings, samples, complications and plan.

Letters and approvals

Respiratory work needs approvals for biologics, CT, home oxygen, sleep studies and pulmonary rehabilitation. The prior authorization letter mode drafts the case; the biologics, CT scan and sleep study pages list what each request should show.

Patients value a clear written plan. Patient instructions can turn your action plan into plain steps: what to take daily, what to do when symptoms worsen, and when to seek urgent care. A sick note during an exacerbation is one more click.

Inhaler checks and education

A large part of a respiratory visit is teaching: inhaler technique, spacer use, what to do in a flare. When you explain it aloud, the note can show that technique was checked and education was given, which matters for any later request for escalation.

Detailed length keeps that counselling; Concise keeps the decision and key numbers.

What ClearPass does not do

Privacy

Clinical note content is not stored on ClearPass servers: it is processed to write the note, then discarded. Consultation audio is transcribed and discarded, never stored. Your note history stays in your browser, on your device, and you choose how long it is kept. Common identifiers are removed from typed and transcribed text before it reaches the AI; that is a best-effort safeguard, so leave names, ID numbers and file numbers out where you can. Processing goes through the AI providers named in our Privacy Policy, which sets out exactly what each one receives and how long it keeps it. ClearPass does not sign HIPAA business associate agreements today.

Questions

Can it read a spirometry printout?

Yes. Photograph the report; the values and interpretation are read into the note. Remove identifiers from the photo first, and check the values against the printout.

Does coughing or a nebuliser in the room affect transcription?

Background noise can reduce quality. Speak clearly and review the draft; the numbers check helps catch misheard figures.

Can it write an asthma action plan?

It drafts patient instructions from the plan you described. Review and adjust it before giving it to the patient.

Will it tell me whether a patient qualifies for a biologic?

No. It documents the criteria you discussed. The clinical decision is yours.

What does ClearPass cost?

The 7-day trial is free, needs no card and simply ends after 7 days, with no automatic charge. After that there is one plan: $78 a month or $858 a year, with every visit transcribed.
