Twofold and ClearPass both offer a week-long free trial with no card and one main paid plan for the individual clinician. Twofold covers a broad range of clinicians, including mental health and allied health, and signs a BAA with every account. ClearPass focuses on medical visits and the letters that follow them. How the offers compare
Twofold's pricing explainer states $69 a month month-to-month or $49 a month billed annually, and its pricing page shows a first-month offer of $19. The 7-day trial needs no card and gives full access: unlimited notes, every clinical template, coding, the AI assistant, and mobile and desktop access. A Group plan is priced by volume.
Twofold's homepage lists the clinicians it serves: primary care, mental health, specialists, nursing, allied health, dentistry, aged care, veterinary medicine and clinical training. Its note formats include SOAP, DAP, BIRP, GIRP, intake, H&P and discharge. ClearPass is aimed at physicians and other medical clinicians, with note structures for SOAP, full nine-section notes, follow-up, new patient, procedure, problem list and your own headings.

Twofold's strengths
- It signs a business associate agreement with every account at no extra cost. ClearPass does not sign one today.
- Native apps on both phone platforms, where ClearPass is a browser app.
- Therapy and behavioural health formats such as DAP, BIRP and GIRP, useful for counsellors and psychologists.
- Its security page states that data is not used to train AI models and that sub-processors handling health data sign BAAs with it. It also says it is undergoing a SOC 2 Type II audit.
- A lower annual price than many tools in this group, if you commit for a year.
ClearPass's differences
- Insurance work from the same visit: a justification letter for prior authorization, and Rejection Analysis for a denied claim.
- Guided case templates across 19 medical specialty groups, alongside the seven note structures.
- A numbers check that flags any value in the note that was never said in the visit.
- An Ask box for plain-language edits, such as a shorter plan or an added section.
- No note text is stored on ClearPass servers; history stays in your browser. Twofold keeps summarised notes in the clinician's workspace until the user removes them.
Which one to try first
If you are a therapist, work in allied health, or your employer needs a signed BAA, start with Twofold. If you are a physician whose time goes on referral letters, approval requests and denials as well as notes, start with ClearPass. Either way both trials are free for a week, so test them on the same kind of visit and compare the edit time.
What ClearPass does not do
- It does not replace your judgement. Every note is an AI draft that you review and correct before it is used.
- It is not a medical device and does not make clinical decisions. It is a documentation aid.
- It does not connect to your EMR. The note is copy-ready text that pastes into any system.
Privacy
Clinical note content is not stored on ClearPass servers: it is processed to write the note, then discarded. Consultation audio is transcribed and discarded, never stored. Your note history stays in your browser, on your device, and you choose how long it is kept. Common identifiers are removed from typed and transcribed text before it reaches the AI; that is a best-effort safeguard, so leave names, ID numbers and file numbers out where you can. Processing goes through the AI providers named in our Privacy Policy, which sets out exactly what each one receives and how long it keeps it. ClearPass does not sign HIPAA business associate agreements today.

Not as named formats. You can set up My own structure with those headings once and reuse it, but ClearPass is built mainly for medical visits.
Why choose ClearPass if it has no BAA or native app?
Choose it if the letters around the visit take much of your time, especially prior authorization and denial appeals, and if copy and paste into your EMR is acceptable.
