An AI medical scribe for orthopedics and MSK
Orthopaedic notes are built on mechanism, function and a structured joint examination, and the approval for an MRI or a joint replacement often depends on how well those were written down. What an orthopaedic note must include
- Mechanism of injury or onset, date, and what the patient could do before and after.
- Pain site, radiation, night pain, locking, giving way, and functional limits such as walking distance or stairs.
- Side, and the dominant hand for upper limb problems.
- Examination: inspection, tenderness, range of motion in degrees, power by grade, neurovascular status, and named special tests with results (Lachman, McMurray, Hawkins, Neer, straight leg raise).
- Imaging with the report's findings and the date: X-ray grade, MRI tear type, fracture pattern.
- Conservative treatment tried, with duration: physiotherapy sessions, analgesia, injections, bracing.
That last item matters more than it looks. Most approval requests for MRI, arthroscopy or replacement ask what was tried first and for how long.
Example: knee pain after a twisting injury
A New patient note, Concise. Invented patient.
Presenting complaint: right knee pain for 7 weeks after twisting while playing football. History: felt a pop, swelled within hours. Now medial joint line pain, catching twice a week, one episode of locking. Unable to run. Completed 6 weeks of physiotherapy with little change. Exam: right knee small effusion, medial joint line tenderness. Range 0 to 115 degrees (left 0 to 140). McMurray positive medially. Lachman firm endpoint. Neurovascularly intact. Imaging: X-ray no fracture, joint spaces preserved. Impression: probable medial meniscal tear, right knee. Plan: MRI right knee. Continue home exercises. Review with the scan.

Letters and approvals
Orthopaedic visits generate more approval requests than most. ClearPass writes a prior authorization letter for MRI, a joint replacement request, or a physiotherapy request from the note you reviewed, using a clinical medical-necessity argument. The same visit can also produce a sick note, a return-to-work or sport fitness letter, or patient instructions for a cast or brace.
Fracture clinics and injection lists
Fracture clinic follow-ups are short and repetitive: pain, function, the X-ray, the cast or splint, and the next step. The Follow-up structure at Concise length suits them well. Several patients can be transcribed in one session and separated, which helps on a busy list.
For joint injections, say the joint, side, drug, dose and volume, and whether ultrasound guidance was used. The Procedure structure places each item where it belongs.
What to check in the draft
- Side, every time: knee, shoulder, hip, and which digit for hand problems.
- Range of motion in degrees and which value belongs to which joint.
- Special tests. Positive and negative are easy to swap when several are done in a row.
- Duration and number of physiotherapy sessions and injections.
- Fracture descriptions taken from the imaging report rather than inferred.

Privacy
Clinical note content is not stored on ClearPass servers: it is processed to write the note, then discarded. Consultation audio is transcribed and discarded, never stored. Your note history stays in your browser, on your device, and you choose how long it is kept. Common identifiers are removed from typed and transcribed text before it reaches the AI; that is a best-effort safeguard, so leave names, ID numbers and file numbers out where you can. Processing goes through the AI providers named in our Privacy Policy, which sets out exactly what each one receives and how long it keeps it. ClearPass does not sign HIPAA business associate agreements today.
Questions
Can it write a note while I examine?
Yes. Call out findings as you go, for example 'Lachman negative, McMurray positive medially', and they go into the examination section.
Can I upload an MRI report?
Yes. Photograph the report; the findings are read into the note. Remove the patient's name and identifiers from the photo first.
Does it do injury claims paperwork?
It drafts a medical report or a letter under Something else from the visit. Forms from third parties still need to be filled in your own system.
Does it suggest CPT codes for procedures?
It suggests ICD-10 and procedure codes where your system uses them. You check and confirm each one.
