What a rheumatology review has to capture

Symptoms since the last visit: early morning stiffness and how long it lasts, which joints are painful or swollen, fatigue, flares and what settled them.
Extra-articular features: skin, eyes, mouth and eye dryness, Raynaud's, chest, bowel, weight.
Joint examination by site and side, tender and swollen counts, and the disease activity score you use, such as DAS28 or CDAI, with the previous value for comparison.
Patient-reported measures you collect, for example a functional questionnaire score or a spinal activity index in axial disease.
Blood results for drug monitoring: blood count, liver tests, creatinine, inflammatory markers, with dates.
Treatment: current DMARD doses, folic acid, steroid exposure and plan to taper, adherence, side effects.
Safety items: screening before a biologic, vaccination status, bone health on long-term steroids, eye screening on hydroxychloroquine, pregnancy plans.

Example: rheumatoid arthritis review

Rheumatology follow-up, Follow-up structure

Interval history: Woman in her forties with seropositive rheumatoid arthritis, on methotrexate 20 mg weekly with folic acid for 9 months. Morning stiffness 90 minutes (was 2 hours). Hands and right wrist painful. No infections. Mild nausea day after dose.
Examination: Tender joints 6, swollen joints 4 (right wrist, right MCP 2 and 3, left MCP 2). No nodules. Chest clear.
Results: CRP 18. Blood count, liver tests and creatinine within range last month.
Disease activity: DAS28-CRP 4.6 (was 5.1).
Assessment: Moderate disease activity despite methotrexate at target dose.
Plan: Discussed adding a biologic. Screening requested: TB test, hepatitis B and C, chest X-ray. Vaccinations reviewed. Continue methotrexate. Blood monitoring every 3 months. Review in 6 weeks with results.

Templates and note structures

Reviewing the draft

Check the joint list. Site and side for every tender and swollen joint must match what you examined; the count must match the list.
Check the score and its inputs. The Numbers check marks a score or a marker value that was never said in the transcript.
Check weekly versus daily dosing. A weekly medicine written as daily is a serious error; read every dose line.
Check the screening list. Before a biologic, make sure every test you ordered is written down and nothing was added that you did not request.

It does not replace your judgement. Every note is an AI draft that you review and correct before it is used.

Privacy

Clinical note content is not stored on ClearPass servers: it is processed to write the note, then discarded. Consultation audio is transcribed and discarded, never stored. Your note history stays in your browser, on your device, and you choose how long it is kept. Common identifiers are removed from typed and transcribed text before it reaches the AI; that is a best-effort safeguard, so leave names, ID numbers and file numbers out where you can. Processing goes through the AI providers named in our Privacy Policy, which sets out exactly what each one receives and how long it keeps it. ClearPass does not sign HIPAA business associate agreements today.

Questions

Does ClearPass calculate DAS28 or other scores?

No. It writes down the score you say or type. Calculate it with your usual tool and say the result aloud or add it to the note.

Can I upload the blood monitoring results?

Yes. Photograph a lab report and the values are read into the note. Photos are not redacted automatically, so crop out names and ID numbers first.

Will it write the biologic approval letter from the clinic note?

Yes, the prior authorization letter is written from the same visit, at Concise, Standard or Detailed length. It uses a clinical medical-necessity argument; check it against the requirements of the plan you are writing to.

What does ClearPass cost?
