An AI medical scribe for urology
Urology patients are often reluctant to describe their symptoms, and the details that matter are numeric: flow rate, residual volume, PSA over time, stone size in millimetres. What the urology note should contain
- Lower urinary tract symptoms split into storage and voiding, with a symptom score (IPSS) and the quality-of-life item.

- Haematuria: visible or non-visible, clots, painless or painful, smoking history, occupational exposure.
- Stones: side, size in mm, location, hydronephrosis, previous episodes, and relevant bloods.
- PSA values with dates in sequence, digital rectal examination findings with estimated prostate size, family history.
- Uroflowmetry and bladder scan results, cystoscopy findings, and imaging with side and measurement.

A New patient note, Detailed. Invented patient.
Referral: PSA 6.8 ng/mL, repeat 7.1 ng/mL six weeks later. No previous PSA. Symptoms: nocturia twice, mild hesitancy. IPSS 9, quality of life 2. No haematuria, no bone pain, no weight loss. Family history: father diagnosed with prostate cancer in his 70s. DRE: prostate estimated 40 mL, smooth, no nodule. Urine dipstick negative. Impression: raised PSA with a benign-feeling gland; density about 0.18. Plan: MRI prostate. Discussed the possibility of biopsy depending on the result. Review with the MRI.
PSA figures and their order are the first thing to confirm. A reversed sequence changes the story.
Urology templates

Letters and approvals
Common outputs are patient instructions for stone prevention or after TURP, a referral letter to oncology or nephrology, and a sick note after a procedure.
Prostate MRI, CT for stones and surgical procedures often require approval. Talking about sensitive symptoms
Erectile dysfunction, incontinence and infertility are hard to discuss. Some clinicians find the conversation goes better when they are not typing, because they can keep eye contact and let the patient finish. The consent step before every transcription is also a natural moment to explain that audio is not kept.
If a patient declines, stop and type or dictate a short summary afterwards. The same structures and letters are available for typed input, and the Urology templates help you remember the questions that matter for each problem, such as pad count or erection quality scores.
Reviewing a urology draft
- Side of stones, testicular findings and renal masses.
- Sizes in mm versus cm, and volumes in mL.
- PSA values, their order, and which is the latest.
- Drug names: tamsulosin versus tadalafil, finasteride versus dutasteride.
- Whether a symptom was denied or simply not asked about.

Privacy
Clinical note content is not stored on ClearPass servers: it is processed to write the note, then discarded. Consultation audio is transcribed and discarded, never stored. Your note history stays in your browser, on your device, and you choose how long it is kept. Common identifiers are removed from typed and transcribed text before it reaches the AI; that is a best-effort safeguard, so leave names, ID numbers and file numbers out where you can. Processing goes through the AI providers named in our Privacy Policy, which sets out exactly what each one receives and how long it keeps it. ClearPass does not sign HIPAA business associate agreements today.
Questions
Will patients speak openly with it running?
You confirm consent every time before transcription starts, and audio is discarded after it is transcribed. Many patients are reassured to hear that nothing is kept as audio.
Can it read a CT KUB report?
Yes. Photograph the report and upload it; the findings are read into the note. Remove identifiers from the photo first.
Does it calculate PSA density?
It will write a figure if you say it. If it computes one, treat it as a draft and check the arithmetic yourself.
Can I use it for urodynamics reports?
Dictate the findings or upload the printed summary and choose a structure. It does not connect to urodynamics equipment.
