What the fertility note needs

Duration of trying, frequency of intercourse, previous pregnancies with outcomes for each partner.
Female partner: age, cycle length and regularity, symptoms of endometriosis or polycystic ovaries, previous pelvic infection or surgery, BMI, smoking, folic acid, cervical screening status.
Ovarian reserve and hormones: AMH, antral follicle count, early-cycle hormones, with dates.
Tubal assessment: method and result.
Male partner: history, medicines, testicular problems, semen analysis with volume, concentration, motility and morphology.
Previous treatment cycles: protocol, response, eggs collected, fertilised, embryos, transfers, outcome.
Plan, options discussed, success rates quoted from your clinic's own figures, risks such as ovarian hyperstimulation and multiple pregnancy, and the couple's decision.

Example: first consultation

Fertility clinic, New patient

Couple: Trying for 2 years. No previous pregnancies together.
Female partner: 34, cycles 26 to 30 days, regular. Dysmenorrhoea 7 of 10 for 5 years. No pelvic surgery. BMI 24. Non-smoker. Taking folic acid. AMH 14 pmol/l. Antral follicle count 11. Tubal patency test: both tubes patent.
Male partner: 36, no medical problems, no medicines. Semen analysis: volume 2.8 ml, concentration 22 million/ml, progressive motility 38%, normal forms 4%.
Impression: Unexplained subfertility of 2 years, possible endometriosis.
Plan: Discussed diagnostic laparoscopy, intrauterine insemination and IVF. Couple prefer to start with ovulation tracking and IUI for 3 cycles. Repeat semen analysis in 3 months. Review after cycle 3.

Guided templates and structures

The Obstetrics & Gynaecology group has guided templates for Fertility / IVF, PCOS, Endometriosis, Recurrent miscarriage and Ovarian cyst / mass, which cover many fertility visits. Male-factor visits work with free text and New patient, or a urology referral.

Many clinics prefer the note split by partner. Save headings such as Couple, Female partner, Male partner, Results, Previous cycles, Plan with My own structure, and every consultation follows the same order. For cycle monitoring visits, Follow-up with Concise length is enough.

Letters and documents

Referral letters to urology for male-factor assessment, or to gynaecology for surgery.
Patient instructions for a treatment cycle: medicines, timings, monitoring appointments, and warning signs of ovarian hyperstimulation.
A letter of medical necessity where a plan requires one for treatment or medicines.
Sick notes around egg collection or surgery.
A letter to the family doctor summarising results and plan.

Reviewing the draft

Check whose result is whose. With two people in the room, make sure every result and history item is under the right partner.
Check hormone units and semen parameters. Units differ between laboratories. The Numbers check marks values that were never said.
Check quoted success rates. The note should hold the figures you gave, not general ones.
Keep sensitive history proportionate. Previous pregnancies and relationships can be sensitive; use the Ask box to shorten what the plan does not need.

What ClearPass does not do

It does not replace your judgement. Every note is an AI draft that you review and correct before it is used.
It is not a medical device and does not make clinical decisions. It is a documentation aid.
It does not connect to your EMR. The note is copy-ready text that pastes into any system.

Privacy

Clinical note content is not stored on ClearPass servers: it is processed to write the note, then discarded. Consultation audio is transcribed and discarded, never stored. Your note history stays in your browser, on your device, and you choose how long it is kept. Common identifiers are removed from typed and transcribed text before it reaches the AI; that is a best-effort safeguard, so leave names, ID numbers and file numbers out where you can. Processing goes through the AI providers named in our Privacy Policy, which sets out exactly what each one receives and how long it keeps it. ClearPass does not sign HIPAA business associate agreements today.

Questions

Does it need both partners' consent?

ClearPass asks you to confirm the patient has agreed. In a couple's consultation, ask both people before you start.

Can I upload hormone and semen analysis reports?

Yes. Photograph a report and the values are read into the note. Crop out names and ID numbers first.

Can it quote success rates for a treatment?

No. It does not supply clinical figures. It writes down the figures you quote from your own clinic's data.

How do I document short cycle monitoring scans?

Use Follow-up with Concise length, or save headings such as cycle day, follicle sizes by ovary, endometrial thickness, medicines and next scan with My own structure. Say the measurements aloud as you scan, or type them in afterwards, and check each one against the scan report.

What does ClearPass cost?
