Using an AI scribe with Tebra: pasting a SOAP draft into the clinical note
Independent practices on Tebra usually chart in its SOAP clinical note, with templates and text shortcuts for the routine parts. ClearPass has no connection to Tebra. It writes a SOAP draft of the visit in plain text, and you paste each part into the matching section of the Tebra note before signing.
The Tebra clinical note
Tebra's help centre describes creating a clinical note from the patient's appointment card on the dashboard. A SOAP note there has sections for the chief complaint and subjective history, medications, allergies, mental and functional status, vitals, objective examination, assessment and plan. The chief complaint fills in from the appointment and active medications from the patient's facesheet.
For most sections Tebra lets you use free-form text, text shortcuts or templates, and the note is finished by clicking Sign. Tebra's own site presents itself to independent practices, with pages for family medicine, primary care, pediatrics, mental health and nurse practitioners.

Because ClearPass writes each heading in capitals on its own line, each section is easy to select and copy on its own. Hyphen bullets paste as plain text.
If a section in your Tebra note does not exist in the SOAP draft, such as mental and functional status, complete it in Tebra as before.
Text shortcuts and templates alongside the draft
Tebra describes custom shortcuts that drop in a full phrase after typing a few characters, and more than a hundred system templates in checklist form. Keep using them for what repeats: standard counselling wording, a normal review of systems, routine follow-up instructions.
Paste the ClearPass text for what changes: the story of this visit and your reasoning about it. If a template already produced a normal examination, paste only the abnormal findings rather than a second full examination.
After pasting, before Sign
- Check each section landed in the right place and nothing is duplicated.
- Confirm every number the ClearPass numbers check marked, especially doses and results.
- Compare suggested ICD-10 codes with the assessment, then choose the diagnosis in Tebra.
- Paste patient instructions or a referral letter drafted in ClearPass where your practice keeps them.
- Then sign in Tebra as usual.
What ClearPass does not do
It does not open Tebra or place anything in the chart for you. It cannot read the facesheet, medications or earlier notes. Tebra offers its own AI note feature; ClearPass is a separate product used by copy and paste. If your practice restricts outside tools, follow that policy. Leave patient names and identifiers out of ClearPass; Tebra already holds them.
What ClearPass does not do
- It does not replace your judgement. Every note is an AI draft that you review and correct before it is used.
- It is not a medical device and does not make clinical decisions. It is a documentation aid.
- It does not connect to your EMR. The note is copy-ready text that pastes into any system.
Privacy
Clinical note content is not stored on ClearPass servers: it is processed to write the note, then discarded. Consultation audio is transcribed and discarded, never stored. Your note history stays in your browser, on your device, and you choose how long it is kept. Common identifiers are removed from typed and transcribed text before it reaches the AI; that is a best-effort safeguard, so leave names, ID numbers and file numbers out where you can. Processing goes through the AI providers named in our Privacy Policy, which sets out exactly what each one receives and how long it keeps it. ClearPass does not sign HIPAA business associate agreements today.
Questions
Does ClearPass connect to Tebra?
No. There is no integration of any kind. You copy the draft and paste it into the Tebra clinical note.
Which ClearPass structure matches the Tebra SOAP note?
SOAP. Its four sections map directly onto Subjective, Objective, Assessment and Plan in the Tebra note.
Should I paste the medication list?
No. Tebra fills active medications from the facesheet. Reconcile them there and paste only what was discussed about changes in the plan.
My practice uses a different Tebra note type. Does that change anything?
Tebra lets you select a different note type from a dropdown. If yours has headings that differ from SOAP, enter them once in My own structure in ClearPass so the draft follows the same order and each part pastes into its own section.
Where do I keep the ClearPass copy after pasting?
ClearPass keeps the note in History in your browser on that device, with optional auto-delete after 7, 30 or 90 days. The signed note in Tebra is the one that counts, so many clinicians set a short auto-delete period.
